Disclosure of Directory Information

No information from School of Medicine records, files, or other data directly related to students, other than public information defined below, shall be disclosed to individuals or agencies outside the university without your written consent except to those with a legitimate educational interest, in response to a lawful subpoena or court order, and to other appropriate persons in an emergency to protect the health or safety of students or others.
The following items have been designated as public, directory information and may be released for any purpose without your prior consent:
  • Name  
  • Telephone number (including pager) 
  • E-mail address
  • Date and place of birth
  • Major
  • Participation in officially recognized activities and sports
  • Dates of attendance
  • Degrees and awards received
  • Institution most recently previously attended
  • Match information (fourth year only per SOM guidelines)
You have the right to withhold disclosure of this public, directory information. Choosing to do so means that any request for this information from non-institutional persons or organizations will automatically be refused, even if disclosure is potentially of benefit to you. The university and SOM assume no liability for honoring your instructions.
